What to do When an Injury Occurs?
How should I report a claim?
1. Sign onto the Online Service Center and submit a First Report of Injury online.
2. Complete an LDOL form 1007, and use one of these three methods to send it to LCTA. Instructions for completing the LDOL form 1007 can be found here.
